First , please upgrade to the latest version of the code (currently 
2004.02.05).  Recent updates to the code fixed some bugs in MPEG-4 video 
streaming.

However, I don't think this will overcome your problem.  It appears that 
your "test.m4v" files are not really raw MPEG-4 video stream files.  (Note 
that ".mp4"-format files - that contain both video and audio - will not 
work.  The "test.m4v" file must be a raw MPEG-4 video stream.)
