OK, the problem was not your file.  Instead, there was a bug in the 
LIVE.COM code's MPEG Program Stream Parser (that just happened to be 
triggered by your file).

***NOTE: Everyone who is streaming MPEG Program Stream files with the 
LIVE.COM code should upgrade to the latest version: 2004.07.22 ***

Once you've upgraded the LIVE.COM code, "testMPEG1or2AudioVideoToDarwin" 
should work with your file.
