Maybe, but such a RTP header extension will be non-standard, and no 
standard receiver (e.g., media player) will understand it.

Instead, just use RTCP.  It's the standard, and media players (e.g., 
QuickTime Player, VLC) understand how to use it to do A/V synchronization.
