Not all of these.  I'm hoping that the existing index file format 
would not need to be changed to support 
H.264-in-MPEG-Transport-Stream files.  If that's the case, then only 
"MPEG2IndexFromTransportStream" would need to be modified.

Yes, but this is done by the existing 
"MPEG2TransportFileServerMediaSubsession" class.  Once again, I'm 
hoping that that would not need to be modified to support 
H.264-in-MPEG-Transport-Stream files.
