It came from a MPEG-2 Program Stream file - 
<http://www.live.com/in.mpg> - which I then converted to a Transport 
Stream using the "testMPEG1or2ProgramToTransportStream" demo 
application (which comes bundled with the "LIVE.COM Streaming Media" 
software distribution).
